The husband in The Shining turned pretty psycho. Maybe him because you could picture him as a real life spouse.

I'm surprised they didn't include either version of Pennywise the clown. He was scarier than Freddy. Freddy was just funny.

In regards to the Pan's Labyrinth creature, that wasn't the first one like that. Here's one from a Destruction album cover years before:




The two concepts are totally different though. In Pan's Labyrinth, the Pale creature was called "not human", being a demon or something I assume. With Destruction, it seems to be an alien that went through a surgery to put its eyes in its hands.
